Payment is due at the time of visit. At this time, only credit card payment is accepted. You will be notified when cash and check payments become available again. We will provide you a credit card receipt for your payment.
We are only in-network with CIGNA. For our Cigna patients, the fee for an appointment will be in accordance with our contracted rates and your in-network benefits with Cigna. As a courtesy, we file electronic claims for our Cigna patients.
For other major commercial insurance companies, patients are considered “self-pay” and “out of network”. We have standard rates established which has not changed since 2014. The fee will be based on the duration of an appointment as a new patient or an established patient. For self-pay patients, whether with or without out-of-network benefits, we can provide you a superbill with which you can file claims and seek reimbursement accordingly.
We do not participate in Medicare and Medicaid insurance. Patients with these insurance plans will have to sign an agreement indicating that they are aware of our having opted-out of these insurance plans, and that they will not file claims with these plans for services rendered at our practice.

We encourage you to verify with your health insurance plan that the provider you want to see is in-network. Once you have ascertained this, please make sure you are aware of your payment responsibility (i.e., whether you have a copay, coinsurance, or deductible).
