A Private Outpatient Psychiatric Practice in Loudoun County

Founded by Dr. Maria Carmen V. Gonzales-Vigilar, MD, MS in 2014

Gonzales-Vigilar Psychiatric Services is a small, intentionally private outpatient psychiatric practice in Ashburn, VA. We have served the Loudoun County community since 2014, and our provider has cared for patients across Northern Virginia, Washington DC, Maryland and West Virginia for far longer. We are a medical practice, not a med spa: psychiatric care here means a careful diagnostic evaluation, evidence-based prescribing and the time to actually listen to what is going on in your life.

Our patients tell us they value three things in particular. First, Dr. Gonzales-Vigilar takes time to understand the full picture before recommending treatment. Second, our Valant patient portal gives you a way to reach your provider between appointments, and we typically respond within 24 to 48 hours. Third, we keep looking for ways to do better, including non-pharmacological technologies like TMS therapy with Exomind and pharmacogenomic testing with Genomind, both of which complement our core psychiatric services.

Three Core Psychiatric Services

These are the foundational services every new psychiatric patient at Gonzales-Vigilar Psychiatric Services will encounter. Most journeys begin with a psychiatric evaluation. From there, ongoing care typically involves medication management, sometimes informed by Genomind gene testing if standard prescribing has not produced a good response.

What to Expect from Psychiatric Care at Our Practice

What to Expect from Psychiatric Care at Our Practice

A two-step approach that respects your time and your story

Step 1: Psychiatric Evaluation. Your first appointment is a comprehensive 60 to 90 minute conversation that covers your current symptoms, medical history, family history, prior medications and goals. By the end of the evaluation you will have a working diagnosis and a treatment plan written specifically for you. Read more about psychiatric evaluation in Ashburn, VA.

Step 2: Medication Management. Most patients return for ongoing follow-up appointments to refine the medication regimen and monitor for side effects. Visits are typically 20 to 30 minutes and recur monthly at first, then less frequently as you stabilize. Learn more about psychiatric medication management.

When standard care is not enough. If two or more medications have not produced a satisfactory response, we have additional tools. Genomind gene testing for medication guidance uses a simple cheek swab to identify which psychiatric medications may work best for your DNA. TMS therapy with Exomind in Ashburn, VA is a non-invasive brain stimulation option for treatment-resistant depression, OCD and other indications.

Therapy collaboration. We do not provide weekly talk therapy ourselves, but we coordinate closely with therapists already in your care or refer you to trusted local providers. Most patients do best with both medication management and therapy.

How TMS therapy with Exomind fits into psychiatric treatment

Some psychiatric conditions, particularly depression, anxiety, OCD and treatment-resistant depression, respond well to TMS therapy with Exomind in Ashburn, VA when medications alone have not been enough.
