Appointments

Office visits for new and established patients are by appointment only and are requested mainly by phone. It is the patient’s responsibility to remember and keep their scheduled appointment.

For new patients, we have a dedicated intake person that will guide you through the process. Patient registration forms can be downloaded from the website and submit by email. Comprehensive clinical forms will also be sent by staff via email. Patients are required to complete and submit the forms no later than 24 hours before the appointment. The appointment will be canceled if these forms are not completed in a timely manner. This policy attempts to balance our psychiatrists’ ability to discuss very important aspects of diagnosis and treatment during the visit while reviewing pertinent clinical records before the visit. 

As a courtesy, our electronic medical system will send a text or email message to patients a few days before their scheduled appointment to remind them of the appointment date and time.

Appointments are scheduled exclusively for a patient.  If patients need to cancel or reschedule an appointment, our office requires at least 24-hour’s notice, otherwise missed appointments or late cancelations will result in the patient being charged a $100 fee.  Patients will receive confirmation of their canceled appointment for their records.
